The Grand Rapids African American Health Institute (GRAAHI) is partnering with Health Net of West Michigan and five (5) Community Partner organizations to assist clients in navigating the healthcare system. A well-qualified team of GRAAHI staff will work in conjunction with outside community organizations that share a common passion to eliminate barriers that exist in health care. Through a joint effort, residents will receive the assistance required to successfully navigate the various health care systems, along with improved access to health and human services agencies.
GRAAHI’s three (3) Community Health Workers (CHWs) will work closely with clients in obtaining the following services:
