Community Health Needs Assessment (CHNA)

Kent County Working Together for a Healthier Tomorrow is a community process designed to identify priority health issues and develop a plan to strategically and collaboratively improve community health. The Kent County Health Department and local hospitals launched this initiative and brought together a broad group of partners concerned about health in our community. This Coalition is working in partnership to achieve our shared vision for health in Kent County.


High quality of life, health, and well-being for all people in Kent County.


The people of Kent County are empowered to achieve lifelong physical, mental and social wellbeing through:

» Equal access to high quality, affordable healthcare;
» A coordinated system of care that is local, preventive, holistic, and patient centered; and 
» An environment that supports healthy living for all.


Kent County Working Together for a Healthier Tomorrow will engage a broad group of community partners to:

» Assess the health status of our community 
We will assess population health, gather community input, and study systems of care in our community to identify health concerns and gaps in services.

» Identify health priorities
We will use our data to identify the most pressing health needs and concerns in our community, and the most significant gaps in services.

» Develop a community-wide health improvement plan 
We will align agency strategic plans with our health priorities and develop a strategic and community-wide plan to work toward our vision.

» Continue to collect health data and engage in collaborative planning 
We will develop and launch an ongoing process to assess the health status of our community and develop community-wide plans to address our most pressing health concerns.

How does GRAAHI contribute to CHNA?

GRAAHI belongs to the Priority 5 workgroup which focuses on reducing disparities among students in Kent County, MI. Our goals and strategies include; coordinating and improving the collection of data related to youth health risk and
protective factors, engaging and empowering youth through community mentoring programs and use
of social/mainstream media, and creating a systematic and integrated holistic assessment tool and resource guide
to be used by community programs targeting youth.
